Your Official Information Act request, reference: GOV-041441 Thank you for your email of 11 July 2025, asking for the following information under the Official Information Act 1982 (the Act):

"i would like the stats for forklift related injuries in NZ over the last 5 -10 yrs including fatalities. this is research for HSSE."

The requested information can be found at this link: https://catalogue.data.govt.nz/dataset/gov-041441-data-for-forklift-related-injuries

This includes a spreadsheet containing data for injuries and deaths related to forklift injuries for the last ten years.

    Please note:
    · Serious Injury claims are identified where the serious injury indicator has been applied. It can take some time for a serious injury claim to be profiled and have the indicator applied, so the claim counts may change over time.

    · ACC is unable to provide comprehensive fatality statistics as data is collected based on claims made. Our data may not represent the total number of fatalities related to accidents for the following reasons:

-Many people are unaware they can make a claim related to an accidental death of a family member. Therefore, ACC only ever receives claims for a subset of accidental deaths.

    - If a person who has an open claim with ACC dies of their injuries, or other causes, ACC is not necessarily notified of this.

Please refer to the notes and caveats in the spread sheet when interpreting the provided data.

As this information may be of interest to other members of the public, ACC may publish a copy of this response on ACC’s website. All requester data, including your name and contact details, will be removed prior to release. The released response will be made available www.acc.co.nz/resources/#/category/12. Thank you for your comment on the info data website on 4 October 2024, received by ACC on 7 October, asking for the following information under the Official Information Act 1982 (the Act):
I am interested in understanding the types and causes of injuries (or worse) related to material
    management generally, and forklifts specifically.

Forklift injury data is attached here: https://catalogue.data.govt.nz/dataset/gov-035698-cause-of-injuries-related-to-material-management-and-forklifts

Table one provides the number of new claims, active claims and active claim costs for forklift related injuries between 1 January 2020 and 10 October 2024.

Tables two and three provide a breakdown of the 10 most prevalent prior activities and accident causes for new claims over the same period.

Table four provides the eight most prevalent primary injury diagnoses for new claims over the same period.

No serious injury or fatal claims have been found for forklift related injuries between 1 January 2020 and 10 October 2024.
Please refer to the notes tab when interpreting the data provided.

We are unable to provide data for injuries related to material management
Our claims data set is largely reliant on the information a person provides on their ACC45 injury claim form. Some of the fields on this form are mandatory to complete and others are not. There is a free-text field where a person can provide a brief description of how their accident happened. This field is not mandatory to complete and not every client does so. It is this field where a client could use the term material management.

We have completed a word search for material management and no claims were found where this was mentioned in the accident description. Therefore, we are refusing this part of your request as finding claims related to material management would require us to manually read through thousands of individual claim files, requiring substantial collation and research. This decision has been made under section 18(f) of the Act.

In doing so, we considered extending our timeframe to respond and charging (as allowed under the Act). However, it was determined that the resources required to extract the data would have a significant impact on the everyday functions of the team(s) involved.

As this information may be of interest to other members of the public
ACC may publish a copy of this response on ACC’s website. All requester data, including your name and
contact details, will be removed prior to release. The released response will be made available
